Soapbox Session: Is a New Approach Needed for Dam Failure Investigations?

An analysis of seven major dam failures over the past 40 years shows that there is no standard method for investigating dam failures. Several investigations can spring-up simultaneously. There have been perceptions of conflict of interest and a corresponding loss in public trust. Investigation reports of the same failure sometimes have little consistency. The technical and human factors that led to the failure are not disseminated to the dam safety industry in a consistent manner. Dam failures are high-profile media events. They create a large burden on state dam safety programs and they can result in multi-year, multi-million-dollar lawsuits.
In the transportation sector, the National Transportation Safety Board has clear authority to promptly and thoroughly investigate major accidents such as plane crashes. Should the dam safety industry adopt a new approach to investigating dam failures?
